New Accommodation

On monday we moved to our 'palace'. Were we are staying for ten weeks, we made a decision to move to a cheaper place. We moved our stuff and checked out of GG hotel. We think we find a good, cosy and safe place to stay. There is à big gate, a lot of rooms, our own kitchen and living room. So we think we can manage for the next ten weeks. Probably two other students from Belgium are joining us this weekend, so we can share the costs.

On monday we had an introduction with Dr. Mahmud. It was a long day, a lot of people were in a meeting. It was not progressing, but at the end of thé day we visited the medical director and the labour ward, meeting the head nurse/midwife.

So on tuesday we arrived at the labour ward, nobody knew about our coming, so it was a suprise. Its difficult to communicate because of the lack of english and they said we had to wait and wait and wait a bit longer. After a while we were asking if we can participate with a midwife, but there was no much work to do. So we helped with cleaning the beds, removing catheters, walking with the doctors, asking a lot of questions, seeing how they did there job and making a lot of social contacts. At 12.00 o' clock we had to go and taking a lunch for two hours. So that was a bit of wasting our time, but we try to do some studywork. In the afternoon luckily there was a woman who got into labour after induction with a cardiac problem. There were already ten other students, but we could observe the first delivery in here. So we know now how they work. While the woman was in labour, she was enduring a lot of pain, because she had female genital mutilation so she was ripped open while the child was born. After the labour they put the child on the table and left It alone, so Marianne stayed with the child and Marleen was observing how they deliver the placenta and start the suture. The medical students interest was changing because there was another woman coming for an amniocentese. Because the father returned we both could observe the suture.

This morning, our second day, there was not much work to do. We follow up a patient with an induction because of IUVF with anencephalia. We counted the contractions manual and we learned a lot about the local procedure. There were sixteen students around a bed of an eclampsia patient. We had to stand on our toos, to see something. Luckily dutch people are taller than the Ethiopian people.

We are thinking about changing to another hospital. They are very creative in using materials. For example they use a glove as a manchet to take a blood sample or set an IV. Also they use a syringe, they cut of the tip and use vacuüm to try to induce the breastfeeding. They also have a protocol for breastfeeding in 1hour after delivery, and stimulating bonding between moms and newborns, it is very nice to see!

We shortage our lunch to 30 minutes instead of two hours so that we can leave earlier. They are very suprised that we don´t go home to eat properly and a lot of people looking very strange if we sit at the side of the road to eat our sandwich.

The afternoon was totally different. At first there was a woman 28 weeks pregnancy suffered from pre-eclampsia and was inducted to deliver a preterm baby. At the time we arrived she was in the second stage of labour. So we transported her fast to the delivery room where Marianne did the delivery of a 900 gram foetus. The foetus had a big head and was putting on the belly of the mum. It was breathing a bit, but it was cyanotic and had no tonus, no crying. It was not clear if we had to try to safe the foetus or it has no purpose. The midwife who was coaching us did´nt knew exactly the status of the patiënt. He tought it was a mors in utero. So we did stimulate the foetus a little bit, and it was making some noises. The foetus had spina bifida. It was not diagnosed before. So we thought in this hospital it does´nt has a chance to survive. After a while the foetus was laying at the rescucitation table where it was left alone and no doctor was around to check the foetus. So marleen was taking care of the foetus while Marianne delivered the placenta. After 15 minutes doctors arrived and they took the baby to the NICU and there they started insufflations after 30 min, it was still gasping. But it did´nt had a chance to survive. After that marleen was assisted a difficult delivery. It was taking a long time. So marleen could cut the umbilical cord and delivered the placenta.

In here there are no fathers present at the delivery, so the woman enduring pain alone but we saw that the fathers are taking care of the woman, helping with urinating next to the bed. So they are very caring and they waiting outside the delivery room till the baby is born. Marianne could do a delivery of a child with shoulderdystocia. At last it was taken over, because the shoulder was really, really stucked. Together we did the Mc Roberts manoeuvre and it worked out. So after all it was a really interesting and impressing day. We can't tell all the things we see, it's too much. So we ended our day in our favorite juice house where we get hugs and kisses from the owner and we really feel like home.
